2022 Flu Season

Due to COVID-19, flu clinics will be minimal.  There will be a drive-through clinic on September 29, 2022.  We encourage all residents to participate in the drive-through.

​

Be prepared with the following:

• Have payment or copy of insurance. 

• Have injection site exposed

• Vaccine consent form (link above) must be printed and filled out BEFORE receiving your flu shot. We will not accept a previous year's consent form. 

General information: Vaccine consent forms are available to print, at the link above. For patients under the age of 18, parents will need to sign the consent form and send with child. (If child has no consent form, then they'll receive NO flu shot.)
